Job Details:

The Clinical Transplant Coordinator is accountable for coordinating all aspects of care for the transplant patient population. Assesses and plans with the patient, caregiver, and multidisciplinary healthcare team to meet needs across the care continuum. Assesses data reflecting the patient's status and appropriately interprets information relative to the patient's age-specific needs. Provides patient and family education on a consistent basis. Provides leadership through community involvement, committee work, and quality improvement and research participation.

This person represents the Tufts Medical Center to the New England Quality Care Alliance (NEQCA) as well as Tufts Medicine physicians to support greater integration of outpatient and transplant follow up. This person tracks performance data and conducts regular team meetings to review results with physicians, APPs and staff; helps to create, implement and follow-up on action plans to further improve patient outcomes and efficiency of post transplant management.

PRINCIPAL DUTIES AND ESSENTIAL FUNCTIONS:

  • Communication and Nursing function
  • Serves at the Tufts Medical Center Transplant Nursing liaison to NEQCA and other physicians for transplant-related matters
  • Maintains detailed current nursing and clinical knowledge of transplant process, patient population and regulatory environment
  • Utilizes tools developed to monitor performance
  • Facilitates collaborative relationships among the various disciplines and departments to ensure effective quality patient care delivery
  • Provides consultation to all new transplant patients and family members regarding transplant care and follow up requirements
  • Participates in department administrative meetings
  • Actively participates in QAPI activities related to kidney transplant
  • Responsible for conducting weekly transplant multi-disciplinary meetings
  • Facilitates post transplant plan of care management and presents casesat multidisciplinary meetings
  • Focus on maintaining current referral sources and building new relationship along with pre-transplant team
  • Provide frequent feedback to referring clinicians about the status of their patients within our program
  • Track post-transplant volumes and network relationships.

Job Qualifications:

EDUCATION:

  • BSN required
  • BLS Certification required
  • ACLS Certification prefered

REQUIRED EXPERIENCE:

  • 2-3years ofexperience in Kidney Transplantation, preferably outpatient experience preferred but not required
  • Experience withEpic desired
  • Knowledgeable in CMS and UNOS regulations
  • LICENSES, ETC
  • Current Massachusetts Registered Nurse License
