Job Details:

The Satellite Registered Nurse (RN), utilizing innovative and flexible solutions, program planning and best practices with an emphasis on standardization and high reliability, is responsible for establishing an environment that fosters teamwork, role competence and proficiency in the satellite clinical setting.

The Satellite RN reports directly to the Associate Chief Nursing Officer and works closely with leadership and a broad spectrum of healthcare providers and administrative personnel to ensure optimal patient care in the satellite clinical setting.

The Satellite RNs primary role is housed off-site in a TuftsMed affiliated organizational setting with the potential to flex on-site as needed to meet patient care needs.

The Satellite RN works in close liaison with the Health Care Team to ensure that patient care is coordinated, comprehensive, and that care transitions are seamless

The Satellite RN practices at the top of their scope and actively develops, coordinates, oversees and administers care to patients in accordance within the framework of team-based patient-centered model of care.

PRINCIPAL DUTIES AND ESSENTIAL FUNCTIONS:

The Satellite RN:

  • Is a leader within nursing providing direction and priority setting in regards to patient care.
  • Develops and maintains collaborative relationships between all members of the Health Care Team inclusive of administrative, clinical, research and inpatient partners thus producing high performing, patient focused teamwork.
  • Acts as a resource for all staff in regards to education, planning, relocating and development of evidence-based, best practice clinical programs pertaining to the specialty or primary care patient population.
  • Is a mentor to and active participate in overseeing all clinical support staffing in the ambulatory clinical setting; delegates responsibilities appropriately and within scope
  • In conjunction with satellite clinical and administrative leadership, participates in coordinating any transition of patient care from the satellite setting to another in collaboration with and maintaining open communication with a wide range of interdisciplinary team members with the goal of achieving an exceptional patient experience.
  • Identifies high risk patients and works with the appropriate services and interdisciplinary team members to achieve the best possible patient care and outcomes.
  • Promotes shared decision making with patients in regards to healthcare; educates the patient and /or family regarding the patients clinical condition, treatment, and their role in their healthcare decisions.
  • Utilizes a variety of telecommunications technologies to assess, triage, provide nursing consultation, and perform follow-up and surveillance of patient status and outcomes
  • Collaborates and actively engages with the care team and practice leadership to identify opportunities and solutions to improve quality, patient access and cost savings.
  • Demonstrates compliance with regulatory requirements i.e. MIPS, fall assessment
  • Participates in satellite quality and safety initiatives as indicated

Job Qualifications:

Possesses and applies the skills and knowledge necessary to provide care to patients throughout the life span, with consideration of aging processes, human development stages and cultural patterns in each step of the care process.

Ability to provide care for a designated group of patients according to unit guidelines and hospital policies, procedures and protocols.

EDUCATION

  • EDUCATION

  • BSN required
  • ADN candidates considered on an individual basis

EXPERIENCE

  • Varies according to unit requirements and needs.

LICENSE

  • Current Massachusetts Registered Nurse License

PHYSICAL DEMANDS/WORKING CONDITIONS

  • Prolonged, extensive, or considerable standing/walking
  • Lifts, positions, pushes and/or transfer patients and equipment
  • Considerable reaching, stooping, bending, kneeling, crouching
  • Frequent exposure to hazardous chemicals, sick patients, bodily substances, noise and possible exposure to radiation, lasers, electric shock, etc.
  • Regularly exposed to the risk of bloodborne diseases and other transmissible infections
  • Contact with patients under wide variety of circumstances
  • Subject to varying and unpredictable situations
  • Handle emergency and crisis situations
  • Subjected to irregular hours
  • May have contact with hazardous materials
